What to bring
For sports physicals: Students should bring their most recent immunization records and list of current medications. Parent or legal guardian must sign the physical form. The sports physicals are offered free of charge at this event
For immunizations: Central District Health Department (CDHD) will provide an immunization record assessment and administer all age appropriate childhood vaccines at this event. Your child’s vaccination status will not impact their eligibility to play sports. This is an opportunity to vaccinate students and educate parents on recommended adolescent vaccines. The Special Event Immunization Consent Form is located at Central District Health Department or on the internet at http://goo.gl/JzwshK and must be completed and returned with students the day of the sports physicals between 1-4 pm.
The form must be
a) entirely completed on both sides, b) signed & dated, and c) returned with a copy of the student’s insurance card.
There is an administrative fee of $20.13 per vaccine. CDHD will bill student’s Insurance/Medicaid for the administration fee(s). If they do not have insurance, they will be billed directly. Sliding fee scale may apply. No one will be denied vaccination due to inability to pay. CDHD staff will assess the student’s immunization records based solely on the records students or families provide. For grades 6-12. This is the only school sports physicals event planned for the 2015 year. Providers will be available from St. Luke's Clinic Trinity Mountain,St. Luke's Clinic Family Medicine and Glenns Ferry Health Center, Inc.All youth are encouraged to see their primary care provider for their annual wellness exam.
